<p/><p/><p><b>Advanced Practice Provider-Hospitalist </b></p><p/><p><b>Position Title:</b> Hospitalist Advanced Practice Provider (APP)</p><p><b>Location:</b> St. George Regional Hospital</p><p><b>Department:</b> Internal Medicine</p><p><b>Position Type:</b> PRN<br/></p><p><b>Job Description Summary</b><br/></p><p>We are a rapidly expanding our Hospitalist team of physicians and advanced practice providers committed to clinical quality improvement and ensuring patients receive the highest quality care at an affordable cost.</p><p/><p>We are looking for an experienced inpatient APP to join our St. George Regional Hospitalist team on an as-needed basis (PRN). PRN providers are required to submit availability for at least two shifts per month. Please note that shifts are not guaranteed in this PRN role.</p><p/><p>The Hospitalist Advanced Practice Provider (APP) will play a critical role in the management of hospitalized patients, with a primary focus on those on general medicine floors. In this role you will work with a multidisciplinary team of doctors, case managers, nurses, dietitians, and pharmacists to provide optimal patient care and outcomes.<br/><br/><b>Key Responsibilities:</b></p><p><br/>Advanced Practice Providers (APPs) are accountable for the provision of patient care at the top of licensure, within their scope of practice. Licensed, credentialed and privileged to perform such duties as:</p><ul><li><p>Conduct comprehensive assessments, including history and physical examinations.</p></li><li><p>Develop, implement, and evaluate treatment plans in collaboration with attending physicians.</p></li><li><p>Manage acute and chronic medical conditions, including initiating appropriate diagnostic and therapeutic interventions.</p></li><li><p>Work closely with multidisciplinary teams, including physicians, nurses, respiratory therapists, and pharmacists, to ensure coordinated patient care.</p></li><li><p>Facilitate communication between patients, families, and healthcare teams, ensuring that patients and their families are informed and involved in care decisions and care transitions.</p></li><li><p>Maintain accurate and timely documentation of patient encounters, treatment plans, and progress notes in the electronic health record (EHR) system.</p></li><li><p>Ensure compliance with hospital policies, regulatory requirements, and best practice standards.</p></li><li><p>Participate in ongoing professional development and continuing education to stay current with advancements in critical care and internal medicine.</p></li><li><p>Engage in quality improvement initiatives within the hospitalist program and hospital.</p></li><li><p>Participate in hospital committees, task forces, and meetings as required.</p></li></ul><p/><p><b>About us:</b></p><p><a href="https://intermountainhealthcare.org/" rel="nofollow noopener noreferrer" target="_blank">Intermountain Health</a> is a model health system by providing extraordinary care and superior service at an affordable cost. Perform delegated tasks after instruction by RN or PT.</li><li>Bathe clients, assist with showers and partial baths as appropriate.</li><li>Assist clients with proper exercises and assist PT in the rehabilitation for client when applicable.</li><li>Help client maintain personal hygiene and assist with all aspects of activities of daily living.</li><li>Answer client questions about self-care techniques and give instructions that will assist family, client, and family/support in providing care.</li><li>Assist client in getting ready for doctor appointments.</li><li>Lift, turn, and weigh clients as appropriate.</li><li>Take and record vital signs as assigned, including blood pressure, temperature, respiration.</li></ul><h4>Qualifications</h4><ul><li><strong>Certified Nursing Assistant (CNA) strongly preferred.</strong></li><li>Must be registered and listed in good standing on the Nurse Aide Registry in the state of employment.</li><li>Minimum of 1 year of experience in a home health or hospice setting.</li><li>Minimum of 1 year of full-time experience in direct client care in an institutional setting (hospital or nursing facility).</li><li>Must have the ability to recognize the needs of people, to maintain good relationships with people, and have the capacity to make meaningful observations and communicate them to the supervisor.<strong> </strong></li></ul><p><strong>If you are interested in this position, please APPLY NOW by completing an online application! </strong></p><h4>Additional Information</h4><p><em>Enhabit Home Health & Hospice is an equal opportunity employer.
