You start you day as a Home Care RN by mapping your visits within your chosen territory, then step into homes to complete SOCs, recerts, routine skilled visits, and discharges-performing thorough assessments, implementing the plan of care, and teaching patients and caregivers in language that sticks; between visits, you capture accurate EMR documentation (assessments, care, orders, signatures) to keep care billable and coordinated; you update and align with physicians, therapists, care coordinators, and admin staff, responding quickly to changes in condition, safety concerns, or non‑compliance to prevent avoidable ED trips; you round out your day by reviewing QA insights and ensuring policies and payer rules are met so your patients keep progressing at home. Productivity expectation of 30 patient visits per week, with an additional per-visit rate of approximately $65–$70 for visits completed above that threshold.
