Conduct specialized care consultations and needs assessment for people living with ALS, caregivers, and family a variety of settings including office visits, phone/email, in clinic, at support groups, virtual home visits, or in person home visits, when appropriate. Reporting to the Care Services Director or Care Services Manager, the Care Services Coordinator will provide comprehensive and professional services, community resources, education, and advocacy to people with ALS, their families, and caregivers in Alaska.

Candidates must reside in Alaska, with preference given to those located in Anchorage, AK*</strong></div><div><br/><br/><strong>POSITION SUMMARY:</strong><br/>Reporting to the Care Services Director or Care Services Manager, the Care Services Coordinator will provide comprehensive and professional services, community resources, education, and advocacy to people with ALS, their families, and caregivers in Alaska.<br/><br/><br/><b>ESSENTIAL DUTIES AND RESPONSIBILITIES:</b></div><ul><li>Respond to persons living with ALS, their families, and caregivers’ inquiries for information on ALS and/or community resources.</li><li>Assist persons living with ALS to register with The ALS Association as well as the ALS National Registry.</li><li>Receive referrals and obtains all necessary patient, medical, demographic information, and documents from referral sources via various communication methods to establish and coordinate a plan of care.</li><li>Assess both physical and psychosocial needs of those living with ALS and families, based on knowledge of ALS, the disease, and its ramifications.</li><li>Conduct specialized care consultations and needs assessment for people living with ALS, caregivers, and family a variety of settings including office visits, phone/email, in clinic, at support groups, virtual home visits, or in person home visits, when appropriate.</li><li>Advocate and assist people living with ALS obtain equipment, services, and care to improve quality of life.</li><li>Consistently maintain quarterly contact/engagement with those living with ALS and/or their caregiver based on assigned area within the territory.</li><li>Connect people living with ALS, caregivers, and families to programs and services provided by The ALS Association.</li><li>Document all pertinent engagements with persons living with ALS, family, caregivers, or other healthcare professionals involved with the delivery of care and support in Salesforce.</li><li>Responsible for educating and informing the general public and professionals about the nature of ALS, its management, related research, and the existence of services of The ALS Association.</li><li>Update supervisor with respect to local community resources and educate those served regarding available resources.</li><li>Facilitate support groups, engage participants, and schedule speakers as appropriate to meet the needs of the group.</li><li>Serve as a member of the ALS Clinic multidisciplinary team and function as the liaison between the clinic and The Association.</li><li>In partnership with supervisor, participate in Care Services program measurement and evaluation.</li><li>Initiate and cultivate relationships with allied health professionals and community social and healthcare organizations.</li><li>Provide professional in-service educational programs to healthcare professionals such as: home health and hospice agencies, Durable Medical Equipment (DME) providers, and Skilled Nursing Facilities.</li><li>Participate in continuing education seminars, workshops, and conferences as available and necessary to gain knowledge on related topics.</li><li>Performs other related duties as assigned by management.</li></ul><div><br/><b>QUALIFICATIONS:</b></div><ul><li>Bachelor’s degree in social work, nursing, public health, or related area. Occupational therapist, physical therapist, speech therapist or direct experience working with the ALS population may be considered.</li><li>Associate's Degree may be considered if combined with 3+ years of experience in a similar role.</li><li>Ability to work independently with little supervision; must be a self-starter.</li><li>Knowledge of ALS a plus, but not required.</li><li>Knowledge of Medicare, Medicaid, patient assessments skills, planning and case management preferred.</li><li>Commitment to organization and highly efficient and conscientious about thorough follow-up.</li><li>Skilled communicator, with superior interpersonal, verbal, and written skills.</li><li>Able to handle confidential information and maintain high confidentiality.</li><li>Excellent organizational and writing skills as well as diligent attention to detail.</li><li>Employee must be able to lift or move 10 pounds, infrequently lift and/or move up to 25 pounds.</li><li>Excellent computer skills. Proficiency with Microsoft Office products and other databases, Salesforce a plus.</li><li>Ability to drive locally and travel nationally on Association business is required.</li><li>Ability to work occasional evenings and/or weekends for Association business and events.</li></ul><br/><strong>PAY TRANSPARENCY:</strong><br/>The ALS Association’s pay range for this position is $22.51 - $27.99 per hour.<p><em>The ALS Association has compensation ranges for various work locations throughout the United States, allowing us to compensate employees competitively and consistently in diverse geographic markets.<strong> The above-listed listed range shows prospective compensation for this role; the appropriate range will be established by what is eventually determined to be the candidate's primary work location.
