What You’ll DoPatient Support· Receive patients through a warm handoff from Patient Development team· Attend doctor visits to support and facilitate communication· Conduct home visits to assess needs and deliver resources· Perform initial setup visits for newly approved patients· Assist with caregiver onboarding and program educationCommunication· Be the main point of contact for ALL patient inquiries and updates· Facilitate communication between all departments to ensure coordinated, patient-centered care· Make courtesy calls to check on well-being and address concerns· Conduct milestone calls to track progress and share timely updatesAdvocacy· Collaborate with providers to ensure personalized, high-quality care· Advocate for patient needs and rights within the organizationWhat We're Looking For.
