We're looking for an experienced RN Case Manager interested in building a long-term employment relationship with our agency by consistently accepting and executing RNCM and recertification visits and weekly medicine setups as needs arise, and providing in-office State Survey support as needed (1-2x/year) in our Perrysburg/Toledo Office, under the guidance of our Northwest Ohio Administrator.
