Job Title: Youth Outreach Case Manager (ECM)
Department: Community Programs
FLSA Status: Non-Exempt
Reports To: CCBHC Program Manager
Hours: Monday-Friday, 8:30 AM to 5:00 PM
Position Summary:
The Youth Outreach Case Manager is responsible for providing enhanced case management services to young adults ages 18-24 who are experiencing significant barriers to stable housing, education, healthcare, and other critical needs. This role involves proactive outreach in the community, home visiting, and the development of individualized service plans that promote stability and self-sufficiency. The position includes regular travel within the community, and company vehicles are provided.

Qualifications:
Language Ability:
Must have high level language skills. Ability to read, analyze, and interpret general business periodicals, technical procedures, Ability to write reports and business correspondence. Ability to effectively present information and respond to questions from managers, clients, customers, and the general public.
Reasoning Ability:
Must have good reasoning skills. Ability to define problems, collect data, establish facts, and draw valid conclusions.
Computer Skills:
To perform this job successfully, an individual should have good working knowledge of Windows operating systems, Microsoft Office Suite - Word, Excel, Outlook, and Internet browsers.
