Now Hiring: Care Coordinator Location: West Allis, WI | Full-Time | Flexible Hours | Hybrid Work Option AvailableJoin our passionate team at PSG/CIP, a dynamic and community-focused social services organization providing innovative mental health programs across eastern Wisconsin! Position OverviewWe are seeking a compassionate and driven Care Coordinator to support individuals with significant mental health and substance use challenges. You'll deliver wraparound, strength-based services that empower clients in their homes and communities, using a recovery-oriented and person-centered approach. Key ResponsibilitiesConduct comprehensive strengths and needs assessments during intake and throughout servicesDevelop and implement individualized Recovery Plans of CareCoordinate and lead team meetings with service providers and support networksProvide crisis intervention and safety planning as neededAttend court hearings, psychiatry appointments, and provide advocacy supportSafely transport clients to appointments when necessaryBuild strong formal and informal support teams Position Details Typical Hours: Monday–Friday, 9:00 AM – 5:00 PM (some flexibility) Travel: Local travel required; mileage reimbursement provided Work Setting: Hybrid/remote scheduling available Pay:• $23.00/hour – Bachelor's Degree• $26.00/hour – Master's Degree What You'll NeedBachelor's Degree in Social Work, Psychology, Criminal Justice, or related field (required)Valid driver's license, reliable daily transportation, and insurance1+ year experience working with individuals with mental health or substance use needs (preferred) Why Work with PSG/CIP?
