Wound/Treatment Nurse - LVN

Pay Range: $28.00-$32.00 per hour, based on experience
Bakersfield Post Acute is seeking a compassionate and experienced Licensed Vocational Nurse (LVN) to join our team as a full-time Wound/Treatment Nurse. This position ensures residents receive effective and efficient wound care and treatments in accordance with physician orders, facility policies, and all applicable federal, state, and local regulations.
The Wound/Treatment Nurse reports directly to the Director of Nursing and plays an essential role in promoting healing, maintaining skin integrity, and providing the highest quality of resident-centered care.
Essential Responsibilities
Perform wound care and other treatments according to physician orders and established care plans
Accurately assess and document wounds, pressure injuries, skin conditions, and residents' responses to treatment
Monitor healing progress and promptly notify physicians and nursing leadership of deterioration, infection, or lack of improvement
Maintain complete, accurate, and timely treatment records and nursing notes
Carry out physician orders for medications, treatments, and resident care
Provide all treatments in a manner that preserves each resident's dignity, comfort, privacy, and self-respect
Communicate regularly with residents and families regarding plans of care and treatment protocols
Collaborate with physicians, nursing, therapy, dietary, and other members of the interdisciplinary team
Ensure appropriate wound-management services and interventions are included in the care plan, including pressure-relieving surfaces and nutritional support
Assist with developing and updating treatment-related care plans
Assist physicians and other healthcare providers during resident visits and examinations
Notify physicians and families of significant treatment-related concerns, including:
Sudden or significant changes in wounds or skin condition
Adverse responses to, refusal of, or unavailability of prescribed treatments
Treatment errors that may present a significant risk
Delays in obtaining medications, equipment, supplies, or services that may affect resident safety
Report significant findings and changes in condition to the RN Supervisor and Director of Nursing
Maintain an acceptable standard of nursing practice and professional conduct
Provide reassurance and compassionate support to residents who may be anxious or distressed
Assist with the orientation and ongoing education of nursing staff regarding wound care and treatment procedures
Participate in facility surveys and inspections as requested
Follow infection-prevention practices and all applicable facility, state, and federal requirements
