Job Type: Full-Time/Part-Time


Compensation: $105,000–$145,000 annually

Our team, Pantheon Health System, LLC, is seeking a Wound Care Physician Assistant or Nurse Practitioner to support our expanding clinical services. This role includes rounding within long-term care facilities in Tulsa and/or Oklahoma City (OKC), with the flexibility to complete charting remotely. We are looking for a provider who shares our commitment to high-quality, patient-centered wound care. To support your work, the owners of Pantheon Health System have developed a physician-designed wound care app that allows providers to monitor, document, and manage wounds with confidence, track healing through a mobile platform, and seamlessly integrate documentation into our EMR. We serve patients who rely on us for compassionate, comprehensive wound care, and we are seeking a licensed Wound Care PA/NP who can deliver skilled clinical assessment and treatment while upholding the standard of care that defines Pantheon Health System.



Responsibilities


  • Examine patients and perform comprehensive wound assessments.


  • Obtain detailed medical histories and document clinically relevant findings.


  • Evaluate, diagnose, and manage acute and chronic wounds, including pressure injuries, diabetic ulcers, and surgical wounds.


  • Develop and implement individualized wound care treatment plans in collaboration with physicians, nursing staff.
  • Perform wound debridement and prescribe wound care therapies and medications in accordance with scope of practice and physician collaboration requirements.


  • Interpret diagnostic tests and clinical data related to wound healing and infection management.


  • Educate patients, caregivers, and facility staff on wound care plans, prevention strategies, and expected outcomes.


  • Maintain accurate, timely, and compliant clinical documentation, including assessments, treatment plans, and follow-up notes.


  • Coordinate care and case management responsibilities across long-term care, skilled nursing, assisted living, and home health settings.

Qualifications



Education:


  • Graduate of an accredited Physician Assistant or Nurse Practitioner program


  • Active, unrestricted PA or NP license in the State of Oklahoma


  • National certification (NCCPA for PA or appropriate NP board certification)


  • DEA registration (or eligibility to obtain)


  • Current BLS certification




Experience: 


  • Minimum of 2 years of clinical experience, preferably in wound or primary care. 
  • Experience in long-term care, assisted living, or home health preferred




Skills


  • Proficiency in wound assessment, treatment planning, and infection management


  • Strong experience with EHR/EMR systems, clinical documentation, and charting


  • Knowledge of wound care products, dressings, and debridement techniques


  • Ability to collaborate effectively with physicians, nurses, and facility staff


  • Proficiency in English, both verbal and written


Physical & Environmental Requirements


  • Ability to see with corrective eyewear and hear clearly with or without assistance


  • May be exposed to infectious or contagious diseases and work in close contact with patients


  • Required to wear personal protective equipment (PPE) as necessary




Physical Demands:


  • Occasional bending, squatting, kneeling, twisting, lifting, carrying, pushing, and traveling
  • Ability to assist with patient mobility as needed, including wheelchair transport
  • Ability to lift and carry up to 15–20 pounds
