Join our exceptional team at Grandview Terrace in Sun City West, AZ, as a Wound Care Nurse!Your Mission: The Wound Care Nurse provides specialized clinical assessment, treatment, and management of resident wounds within the skilled nursing facility, including pressure injuries, surgical wounds, and chronic conditions such as diabetic and vascular ulcers. This position partners with the interdisciplinary care team to develop individualized wound care plans, monitors healing progress, and ensures wound care practices align with evidence-based standards and regulatory requirements. The Wound Care Nurse also serves as a clinical resource, educating nursing staff, residents, and families on wound prevention and care.Why Choose Us?Great Place to Work: Sun Health has been certified as a Great Place to Work in 2021, 2022, and 2023.National Recognition: We secured a top 10 spot on the Fortune Best Workplaces for Aging Services list in 2020 and 2021.Platinum Award Winner: Our dedication to health and safety earned us Platinum awards from the Healthy Arizona Worksites Program in 2019, 2020, and 2021.Key Responsibilities:Wound Assessment & Clinical CareConducts comprehensive wound assessments for residents with pressure injuries, surgical wounds, diabetic ulcers, and other chronic or acute wounds, determining appropriate treatment interventions.Performs wound care procedures, including debridement, dressing changes, and application of advanced wound care products, in accordance with physician orders and evidence-based protocols.Monitors wound healing progress and adjusts treatment plans based on clinical response, escalating to the physician or wound care specialist as needed.Identifies residents at risk for skin breakdown and implements preventive interventions to reduce pressure injury incidence.Supports the admissions process by verifying incoming skin assessments, identifying skin concerns at time of admission, and coordinating with the care team on any changes in condition.Care Planning & Interdisciplinary CollaborationPartners with physicians, nurse practitioners, dietitians, and therapy staff to develop and update individualized wound care plans.Participates in interdisciplinary care conferences to communicate wound status, treatment progress, and care plan updates.Coordinates with hospice, home health, or outside wound care specialists when residents require services beyond the scope of the facility.Documentation & Regulatory ComplianceMaintains accurate, timely documentation of wound assessments, treatments, and outcomes in the electronic health record, in accordance with facility policy and regulatory requirements.Ensures wound care practices comply with CMS, state, and accreditation standards, supporting survey readiness and audit preparedness.Tracks and reports wound-related quality indicators, including pressure injury incidence and healing outcomes, to support facility QAPI initiatives.Staff Education & Wound PreventionProvides ongoing education and training to nursing and certified nursing assistant staff on wound prevention, positioning, skin integrity, and proper wound care techniques.Serves as the facility's clinical resource for wound care best practices, staying current on evidence-based treatment modalities and products.Educates residents and families on wound care, prevention strategies, and expected healing timelines.Skills and Knowledge:Strong clinical knowledge of wound etiology, staging, and evidence-based treatment modalities, including pressure injuries, diabetic ulcers, and surgical wounds.Proficient in wound assessment, debridement techniques, and application of advanced wound care products and dressings.Working knowledge of CMS and state regulatory requirements related to skin integrity and wound care in skilled nursing settings.Proficient in Microsoft Office 365 (Excel, Word, PowerPoint, Outlook, Teams) and electronic health record systems such as PointClickCare or comparable platforms.Strong assessment, critical thinking, and clinical decision-making skills.Excellent communication skills to collaborate with interdisciplinary teams, residents, and families.Ability to educate and train staff on wound prevention and care best practices.High level of accuracy, attention to detail, and commitment to resident safety and dignity.Your Qualifications: Current, unencumbered LPN or RN license in the state of Arizona.Wound Care Certification (e.g., WCC, CWCN, or WOCN) preferred.Minimum 1-2 years of nursing experience required; wound care or skilled nursing/long-term care experience strongly preferred.Demonstrated experience managing complex wound care cases in a healthcare setting.Current CPR certification required.Possess current or has the ability to obtain a Level 1 Fingerprint Clearance Card is required.Employee Benefits:Comprehensive health benefits (Medical, Dental & Vision).401k with immediate vesting and up to 4% employer match.Tuition Reimbursement.Exclusive discounts through The Employee Network.Access to the Employee Association, supporting team members in need.Equal Employment Opportunity: At Sun Health, we are committed to fostering an inclusive workplace where everyone is treated with respect and dignity.
