Job Description

Are you interested in working with one of top medical groups in the country? TeamHealth is seeking a quality-driven nurse practitioner or physician assistant to join our post-acute care team in Mt. Pleasant, Michigan. This is a full-time opportunity (5 days/week) rounding in skilled nursing facilities, with excellent support and training, autonomy, and earning potential. Compensation is fee for service with an estimated compensation range of $115,000 - $150,000 annually and no cap on productivity income potential.

Our innovative population health data reports will guide you towards optimal and timely care for our patient population. The medical practice is part of TeamACO, a dedicated long-term Accountable Care Organization participating in the Medicare Shared Savings Program for our clinicians, which could result in you receiving a portion of the shared savings if certain criteria are met.

Key Responsibilities

  • Patient Assessment: Conduct thorough, detailed evaluations of wound size, depth, drainage, and underlying contributing factors (e.g., poor circulation, diabetes)
  • Treatment Implementation: Execute physician orders and evidence-based protocols, including wound cleaning, debridement (removal of dead tissue), dressing changes, and operating specialized therapies (like negative pressure wound therapy or hyperbaric oxygen)
  • Care Planning: Design and update individualized treatment and skin prevention plans in collaboration with the multidisciplinary healthcare team
  • Education: Teach patients and their caregivers proper home care techniques, hygiene, and preventative measures
  • Staff Mentorship: Act as a clinical resource by training junior nurses and assistive personnel on current wound care management theory
  • Documentation: Record assessments, wound progress, and interventions to ensure continuity of care and proper compliance for insurance reimbursement

Qualifications

  • NP or PA license (State of Michigan) and DEA
  • Experience in wound care post-acute, acute, ED, or clinic settings preferred, but not required
  • Wound care certification strongly preferred but could consider candidate without who has strong wound care experience
  • Strong clinical foundation and knowledge of healthcare regulatory standards
  • Excellent communication and interpersonal skills
  • Collaborative, team-oriented approach with a positive outlook
