    Wellness Coordinator

    Are you passionate about helping others live healthier lives? Join our team as a Wellness Coordinator, where your outreach and care coordination will directly support patients in accessing vital preventive services and improving their overall well-being.

    What You'll Do

    • Be the voice of wellness: Make outreach calls to patients, guiding them through eligible preventive care services like Medicare Annual Wellness Visits.
    • Empower healthier choices: Educate patients on the importance of preventive care and help close care gaps that impact long-term health.
    • Collaborate with care teams: Work side-by-side with physicians, nurses, and staff to create a seamless, integrated approach to patient care.
    • Drive quality improvement: Support primary care practices in achieving better outcomes through documentation, process enhancements, and occasional presentations at staff meetings.
    • Support care management: Assist with general office tasks, secretarial duties, and population health initiatives that make a real difference in our communities.

    What We're Looking For

    • A positive attitude and a team-first mindset.
    • Strong communication skills-especially over the phone and via text.
    • Comfort with technology, including Microsoft Office and electronic medical records.
    • Ability to multi-task, prioritize, and stay organized in a fast-paced environment.
    • A basic understanding of medical terminology is a plus.

    Qualifications

    • Education: High school diploma or equivalent required; Associate Degree preferred.
    • Experience: Prior clerical experience in a healthcare setting is preferred.
