$1,000 Sign on Bonus!

  • $250 after 30 days of employment
  • $250 days after 60 days of employment
  • $500 after 90 days of employment

Location: The Doliver of Tanglewood, an Independent Living and Assisted Living with Memory Care senior community.  

Pay Rate: $35.00/hr.

Full Time Schedule: 5 on 2 off

Morning and afternoon shifts

Availability: Must be available to work weekends and holidays 

Essential Duties and Responsibilities:

  •    Administer medications safely and accurately per physician orders and regulatory standardsOversee medication aides and conduct monthly medication reconciliation and audits
  •    Coordinate daily staff assignments to ensure consistent resident careCommunicate effectively with physicians, families, and care teams regarding resident needs
  •    Provide direct care including ADLs: bathing, dressing, grooming, toileting, mobility, and incontinence supportRespond to medical and behavioral emergencies with sound clinical judgment
  •    Complete clinical documentation including assessments, evaluations, and service plansDocument incidents and communicate promptly with leadership
  •    Monitor care team documentation for accuracy and completenessSupport orientation and ongoing training of care staff
  •    Assist with scheduling in collaboration with leadershipAttend required in-services and continuing education
  •    Maintain professional appearance and hygiene- Promote respectful and effective communication across teams and families\
  •    Perform other duties assigned to support resident care and operational excellence

Qualifications:Current and valid LVN/LPN license in the state of employment

  •    Basic Life Support (BLS) / CPR certificationMinimum 6 months to 1 year experience in Assisted Living or Long-Term Care
  •    Proficient in Electronic Health Records (EHR) and basic computer skillsStrong verbal and written communication skills
  •    Demonstrated critical thinking and problem-solving abilitiesKnowledge of HIPAA regulations and compliance standards
  •    Ability to respond with patience and understanding to dementia-related behaviorsPhysical ability to perform essential job functions with or without reasonable accommodation
