Description
A nonprofit services organization in New York City is seeking a new Wellness Coach to join their team in Brooklyn, connecting people to necessary community services for recently released individuals.
About the Opportunity:
Schedule: Monday to Friday
Hours: 9am to 5pm
Setting: Supportive housing program
Responsibilities:
Providing case management services
Developing and monitoring support plans
Assisting with problem-solving and crisis intervention
Accompanying consumers to appointments
Acting as an advocate for consumers
Performing other duties, as needed
Qualifications:
1+ year of related work experience
High School Diploma / GED
Solid assessment and documentation skills
Patient oriented
Great interpersonal and communication skills
Strong attention to detail
Highly organized
Desired Qualifications:
3 years of experience with Behavioral Health populations
Bachelor's Degree
Experience with Scattering Site settings
Solid understanding of Supportive Housing dynamics
Experience with AWARDS
