Essential Functions

Lead, supervise, and mentor an interdisciplinary team responsible for assessment, care planning, and service coordination across the continuum of care for older adults (60+) and adults (18+) with physical disabilities. Support staff in their important work of assisting older adults with physical disabilities to live and thrive in their homes and communities.

Oversee the daily operation of core ADRC services, including:

  • Information and Referral
  • Community Outreach
  • Screening and Intake
  • Options Counseling
  • In-home assessments and reassessments
  • Care management and supportive counseling

Develop and implement internal procedures to ensure services are delivered in a whole-person-centered, compassionate manner and in compliance with County, State, and Federal ADRC standards.

Supervise care managers by providing role-modeling, mentoring, guidance, consultation, direction, and problem-solving support to staff.

Serve as a member of the Office on Aging and Disability Services management team, contributing to program planning, policy development, quality assurance, and service evaluation.

Ensure that vulnerable residents receive coordinated, high-quality services that support independence, safety, and dignity.

Guide frontline staff and maintain strong program oversight.

Qualifications

Essential:

  • Master's degree in social work with a Licensed Clinical Social Worker (LCSW) in good standing.
  • Strong leadership, organizational, communication, and problem-solving skills.
  • Ability to supervise and mentor staff and manage program operations.
  • Ability to read, write, and communicate clearly in English.
  • Proficiency with computers and standard business software, including Microsoft Office Suite and web-based databases.
  • Valid New Jersey driver's license and ability to travel independently to meetings, community locations, and client residences.

Preferred:

  • Two or more years of supervisory experience in case management or care management programs, preferably serving older adults or individuals with disabilities.
  • Three to five years of experience working with older adults.
  • Experience conducting client assessments and developing care plans.
  • Experience designing or implementing community programs.
  • Knowledge of community mental health services and community resources systems.
  • Familiarity with Medicaid Waiver programs and long-term care services systems.
  • Experience with budget preparation and program management.
  • Ability to work effectively in a fast-paced environment while maintaining high attention to detail.
  • Strong public speaking and stakeholder communication skills.

Salary/Pay Rate:

  • $65,000 - $82,250/year
  • 35 hours/week
