Job Summary:

We are seeking a compassionate and skilled Weekend LPN (Licensed Practical Nurse, Visiting Nurse) to join our team at Hartford HealthCare at Home. As a Weekend LPN, you will provide high-quality patient care and support to our patients in the comfort of their own homes.

Responsibilities:

• Provide direct patient care, including administering medications, monitoring vital signs, and performing wound care • Develop and implement individualized care plans to meet the unique needs of each patient • Collaborate with interdisciplinary teams, including physicians, social workers, and other healthcare professionals • Educate patients and their families on proper care and management of chronic conditions • Document patient information accurately and thoroughly

Requirements:

• Current LPN licensure in the state of Connecticut • 1+ year of experience in a home healthcare setting • Strong communication and interpersonal skills • Ability to work independently and as part of a team • Basic life support certification
