Support Perry Family Free Clinic's mission to improve access to healthcare for underserved and uninsured residents, particularly Black men in Dane County, by increasing access to primary care, preventive health services, health screenings, chronic disease management, and community-based health education through both clinic-based and community outreach initiatives. Member Duties : Connect underserved residents to healthcare via outreach, referrals, and collaboration with social workers, community partners, and local organizations. Assist with patient registration, intake, appointment coordination, follow-up calls, reminders, and patient navigation. Provides health education resources, helps facilitate community health screenings under staff supervision, and supports special initiatives including Operation Diabetes and the Perry Family Free Clinic Barbershop Clinic. Assists with outreach events, volunteer activities, participant registration, and connecting community members to ongoing care. Maintains accurate records of services, referrals, and outreach activities while protecting confidentiality. May transport patients in agency vehicles in accordance with organizational policy. Maintains regular communication with supervisors and staff and complies with all AmeriCorps, host site, confidentiality, professional conduct, and prohibited activities requirements.
