Job Summary
We are looking for a DIRECT CARE WORKER  to join our team! You will directly work with Members with a disability, following a one-on-one care plan in our DDD Group homes. You operate with a big heart, not only providing personalized and attentive care, but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets the member's needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Group Home Experience and working with Behavioral members is a plus. 
Responsibilities 
  • Assist patients with daily activities such as moving in and out of  the community, baths, or automobiles
  • Care for Members  by changing bed linens, doing laundry, cleaning the home, or assisting with personal care
  • Maintain records of member's care, condition, progress Note, or Incidents to report and discuss observations with the supervisor 
  • Build relationships with Members  by conversing or reading aloud to help keep them mentally healthy and alert 
  • Give medications and drive them to Urgent care, Hospitals if needed 
  • Engage Members  in exercises or other activities 
  • going through members centered plan to learn about the member you are to care for.
Qualifications
  • Graduated from an accredited Home Health Aid program
  • High School Diploma or GED 
  • One-year prior professional experience 
  • Driver’s license required 
  • CPR certification required 
  • Prevention and support 
  • Article 9 
  • first Aid and CPR
  • Social Security Card 
Compensation: $14.35 - $15.00 per hour
