    Position Summary
    • Oversee transitional housing opportunities for individuals diagnosed with a severe and persistent mental illness.
    • Review clinical information, determine level of care, authorize treatment.
    • Work on a team to actively coordinate ongoing client care and discharge planning to transition to less restrictive settings.
    • Weekly case staffing with providers and clients.
    • Daily collaboration in a fast-paced team-centered environment.
     
    What We Value in our next Triage Clinician
    • Bachelor’s or master’s degree in counseling, social work, or other mental health related field; and current Ohio licensure. LSW, LISW, LISW-S, LPC, LPCC, and LPCC-S are all desirable.
    • Clinical assessment skills, understanding diagnostic criteria and symptom presentations outline in the DSM-5.
    • Working knowledge of the Hamilton County behavioral health system of care preferred.
    • A strong alignment with trauma-informed, person-centered care.
