Key Responsibilities
- Evaluate and treat patients with speech, language, and swallowing disorders in a long-term care setting
- Develop and implement individualized treatment plans based on accurate patient assessments and prognosis
- Communicate effectively with patients, medical staff, and interdisciplinary care teams
- Document patient progress and maintain records using Epic/EMR software
- Apply knowledge of the PDPM reimbursement system to support appropriate care planning and documentation
Required Qualifications
- Graduate of a college or university accredited by the American Speech and Hearing Association with a degree in Speech Language Pathology
- Current New York State license as a Speech-Language Pathologist
- Eligible for or holds the Certificate of Clinical Competence in Speech Language Pathology (CCC-SLP) from ASHA
- Long-term care (LTC) experience required
- Candidates living within 75 miles of the facility are welcome to apply; local rates will apply
Preferred Experience / Skills
- Proficiency with MS Office and Epic/EMR software
- Working knowledge of the PDPM reimbursement system
- Strong assessment skills with the ability to guide patients toward achieving their fullest potential
