Key Responsibilities
- Evaluate and treat patients with speech, language, cognitive, and swallowing disorders
- Develop and implement individualized treatment plans based on patient needs and goals
- Provide services across multiple facilities within the same city
- Document patient progress and maintain accurate clinical records in accordance with facility and regulatory standards
- Collaborate with interdisciplinary care teams to support optimal patient outcomes
Required Qualifications
- Active Speech Language Pathologist (SLP) license in the state of Illinois
- Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from ASHA preferred or eligibility required
- Ability to work across multiple nearby facilities within the same city
- Strong clinical documentation and communication skills
Preferred Experience / Skills
- Prior experience in a skilled nursing or long-term care setting
- Experience managing a caseload across multiple sites
- Familiarity with dysphagia evaluation and treatment protocols
