Job Summary (Speech-Language Pathologist - Skilled Nursing Facility)
- Assess, diagnose, and treat residents with speech, language, cognitive-linguistic, and swallowing disorders.
- Develop and implement individualized treatment plans to promote functional independence and safety.
- Provide direct therapy services targeting communication, cognition, and safe swallowing.
- Perform and interpret swallow assessments; make diet modification recommendations as needed.
- Document all evaluations, treatments, progress notes, and discharge summaries per regulations.
- Educate residents, families, and staff on therapy plans, communication strategies, and swallowing safety.
- Participate in care planning and interdisciplinary team meetings to support resident-centered goals.
- Monitor and adjust treatment plans based on ongoing resident progress.
- Ensure compliance with professional, legal, and regulatory standards.
- Supervise Clinical Fellows and/or Speech-Language Pathology Assistants as required.
- Work collaboratively in a skilled nursing/long-term care environment, supporting individuals with complex medical needs.
