Job Summary (List Format):
- Assess, diagnose, and treat communication, cognitive-linguistic, and swallowing disorders in skilled nursing facility residents.
- Develop and implement individualized treatment plans to promote independence, safety, and quality of life.
- Provide direct therapy services targeting speech, language, cognition, and swallowing functions.
- Perform and interpret swallowing assessments and recommend diet modifications.
- Document evaluations, treatments, progress, and discharge summaries in compliance with regulations.
- Educate residents, families, and staff on therapy plans, communication strategies, and safe swallowing techniques.
- Collaborate with interdisciplinary care team and participate in care planning meetings.
- Monitor resident progress and adjust treatment plans as needed.
- Ensure compliance with professional, legal, and regulatory standards.
- Supervise Clinical Fellows (CFYs) and/or Speech-Language Pathology Assistants (SLPAs), if applicable.
