Participate in the development of speech therapy plans of care for patients. Make visits to patients for initial and ongoing assessments to identify the patient’s level of functioning and ability to communicate, including OASIS assessments at appropriate time points. Communicate with the physician before the start of care and as needed. Implement the plans of care. Provide education and instruction to team members, patients, and families. Document care and submit paperwork accurately and timely to update and maintain patient medical records. Preferred skills include experience in acute care and patient assessment. Skills related to education and instruction as well as medical documentation are beneficial.
