Description
Key Responsibilities
Provide speech-language pathology services to patients in a home health setting
Complete OASIS assessments in accordance with home health documentation requirements
Develop and implement individualized treatment plans based on patient evaluation and clinical findings
Maintain accurate and timely clinical documentation using required charting systems
Collaborate with interdisciplinary team members to coordinate patient care
Float to other locations as needed based on facility needs
Required Qualifications
Active Virginia Speech Language Pathologist license required; must be in hand at time of submittal or provide proof of compact license
OASIS experience required
Home health experience required
Specialty experience within the last year required
One supervisory reference required at time of submittal
Local candidates are welcome and accepted at the same rate
Preferred Experience / Skills
Experience with electronic health record documentation in a home health environment
Strong organizational and time management skills for managing a home-based caseload
Familiarity with interdisciplinary care coordination in a home health setting
