Conditions may include issues resulting from premature birth, birth injuries, injury, disease, or other causes. Synthesizes data to establish a plan of care through evidence-based practice interventions and goals appropriate to each patient’s age and development. Interacts and collaborates with a variety of professionals to implement and assess the intervention program. Completes other duties as assigned. The selected individual is expected to exhibit high-functioning capabilities and strong self-motivation. A proactive, go-getter approach to all assigned responsibilities is essential. The contractor must consistently demonstrate initiative, independence, timely, and quality care for patients.
