Job Summary (List Format) for Speech-Language Pathologist (SLP) in Skilled Nursing Facility:
- Assess, diagnose, and treat communication, cognitive-linguistic, and swallowing disorders in residents.
- Develop and implement individualized treatment plans to promote functional independence, safety, and quality of life.
- Provide direct therapy services for communication, cognitive-linguistic functioning, and swallowing safety.
- Perform and interpret swallow assessments and recommend diet modifications as needed.
- Document evaluations, treatments, progress notes, and discharge summaries per regulatory and facility guidelines.
- Educate residents, families, and staff on therapy plans, communication strategies, and safe swallowing techniques.
- Collaborate with interdisciplinary care team and participate in care planning and meetings.
- Monitor resident progress and adjust treatment plans based on outcomes.
- Ensure compliance with professional, legal, and regulatory standards.
- Supervise Clinical Fellows (CFYs) and/or Speech-Language Pathology Assistants (SLPAs) as required.
- Work primarily in a skilled nursing/long-term care facility with residents who may have complex medical needs.
- Foster a collaborative and team-oriented therapy environment.
