Position Overview

Tide Health Group is seeking an experienced Speech-Language Pathologist (SLP) for a 13-week Long Term Care assignment in Norman, OK. The SLP will evaluate and treat patients with communication, cognitive, and swallowing disorders while developing individualized plans of care and providing evidence-based rehabilitation services.

Key Responsibilities
Screen and evaluate patients with communication, cognitive, and swallowing disorders.
Develop and implement individualized plans of care.
Perform standardized assessments and utilize evidence-based clinical practices.
Monitor patient progress and modify treatment plans as appropriate.
Evaluate treatment outcomes and make referrals based on clinical findings.
Provide counseling and education to patients, families, caregivers, and other service providers.
Communicate patient progress, barriers, and treatment plans with supervisors and interdisciplinary team members.
Complete discharge summaries and maintain accurate clinical documentation.
Accurately document treatment minutes and required patient information.
Participate in patient care conferences, utilization review, rehabilitation meetings, and other required meetings.
Participate in required facility/company training, in-services, and conferences.
Obtain necessary DME and supplies according to clinical and regulatory requirements.
Supervise and support CFY or student Speech-Language Pathologists when applicable.
Practice in accordance with ASHA standards, Oklahoma regulations, professional ethics, and applicable practice requirements.
Required Qualifications
Minimum 1 year of SLP experience
LTC or Rehabilitation experience required
Active Oklahoma SLP license required and must be documented
CCC-certified SLP required and must be documented
New graduates may be considered only if they have Level II clinical experience; Level I observation-only experience does not qualify.
3 years of experience maintaining an active CCC is preferred
One professional reference accepted at time of submission.

Patterns Hiring Job ID #70245. Pay package is based on 8 hour shifts and 40.0 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
