Description

American Traveler seeks a Speech-Language Pathologist for a home health position requiring CCC certification and Home Health experience with the HomeCare HomeBase EMR.

Details

  • Home health-based position covering parts of Anson and Union counties
  • Caseload assigned based on a point system, averaging 5-6 points per day
  • Adult and geriatric patient population
  • Common diagnoses include CVA, TBI, dysphagia, apraxia, speech and cognition impairment, cancer, and aphasia
  • Requires use of HomeCare HomeBase (HCHB) EMR
  • Day shift schedule, Monday through Friday, 8:30am to 5:00pm
  • No weekend rotation, on-call, or holiday requirements
  • Floating required as part of standard home health duties

Requirements

  • Active state license required, if applicable
  • BLS certification required
  • Master's degree or higher in Speech-Language Pathology
  • Certificate of Clinical Competency (CCC) required
  • Minimum 2 years of experience as an SLP
  • At least 3 months of Home Health experience required (internship experience accepted)
  • Experience with HomeCare HomeBase (HCHB) EMR required, no exceptions
  • Valid driver's license, vehicle, and proof of insurance required
  • Last 4 digits of SSN and full date of birth required for consideration
  • Minimum of 1 professional reference dated within the past year, from a Supervisor, Manager, Director, or Charge RN

Additional Information

  • Perform swallow studies and evaluate care plans to identify aspiration risks
  • Determine appropriate diets, including soft or mechanical options, based on patient needs
  • Provide patient and family education on swallowing and communication disorders
  • Collaborate with an interdisciplinary care team during weekly Care Consult meetings on Tuesdays
  • Access to another SLP by phone for case support as needed
  • Supervisor, manager, and scheduler support available
  • Orientation includes New Employee Orientation (NEO) and one week shadowing an SLP teammate
  • Light gray scrub color required
  • Parking availability depends on patient housing location
  • First-time travelers will not be considered for this role
  • Returning employees must be separated from any AHSE facility for at least one year after working a permanent position
  • Candidates must live outside a 50-mile radius of the facility to be eligible
