This role involves evaluating, rehabilitating, and developing treatment plans for patients with communication, cognitive, or swallowing disorders. Job Details
Position Description: Effectively screens, evaluates, and rehabilitates patients with communication, cognitive, or swallowing disorders. Develops appropriate care plans based on standard assessments and evidence-based practices. Monitors patient progress, adjusts treatments as needed, and evaluates treatment outcomes. Provides consultation to patients, families, caregivers, and other healthcare providers. Communicates progress and barriers with the healthcare team and submits comprehensive discharge summaries. Documents all regulatory and clinical information accurately in the electronic system and maintains patient records. Participates in meetings such as patient care conferences, utilization reviews, and rehabilitation meetings. Supports regulatory and clinical requirements by obtaining necessary supplies and supervising CFY or student Speech-Language Pathologists. Adheres to ASHA and state practice acts, codes of conduct, and ethical standards.
Required Qualifications: Massachusetts State License, CCC certified Speech-Language Pathologist, 1 year of experience (new grads with Level II experience accepted), experience in LTC or rehab settings, coursework or certification as applicable, Bachelors or masters degree in speech-language pathology, Schedule: Days, 8 hours per day, minimum 40 hours per week.
