Responsibilities
- Provide services to patients and their families to address needs incidental to hospitalization.
- Offer counseling, support, and assistance in developing transition plans for discharge.
- Provide direction for routine non-medical problems of patients and their families.
- Assist with planning and post-discharge activities with outside agencies, nursing staff, and relatives.
- Collaborate with the care team to coordinate discharge planning and follow-up resources.
Requirements
- Ability to work in a hospital setting and deliver patient- and family-centered social work services.
- Strong communication and counseling skills to support patients and families through hospitalization and discharge transitions.
- Experience or demonstrated ability in discharge planning and care coordination with external agencies and clinical staff.
