Job Summary:

We are seeking a qualified Licensed Master Social Worker (LMSW) to provide behavioral health social work services in an acute care setting. The Social Worker will assess patients’ psychosocial needs, provide therapeutic interventions, coordinate treatment and aftercare plans, and support safe and appropriate discharge planning.

Job Responsibilities

Conduct comprehensive psychosocial assessments at admission and throughout treatment.
Identify and prioritize patients’ psychosocial problems and develop individualized treatment and aftercare plans.
Serve as the primary therapist for assigned patients.
Provide therapeutic interventions and evaluate patient responses to treatment.
Collaborate with patients, families/significant others, physicians, and multidisciplinary healthcare professionals.
Communicate significant social and emotional factors affecting patients’ mental health to the healthcare team.
Coordinate and manage discharge planning for assigned patients.
Arrange appropriate follow-up services and community resources.
Advocate for patients when coordinating with hospitals, community agencies, and outside organizations.

Requirements

Current New York LMSW license required.
Minimum 2 years of relevant social work experience in a Psych Hospital or Beahvioural Health Unit.
Flu vaccine required — no exemptions or exceptions.
