Job Summary
Occupational Therapist, Skilled Nursing Facility Missouri. The Occupational Therapist evaluates patients with cognitive and physical functioning disorders and develops individualized plans of care. The role provides evidence-based rehabilitation services, monitors treatment outcomes, supports discharge planning, and collaborates with patients, families, caregivers, and interdisciplinary team members.
Job Details
- Responsibilities: Screen and evaluate patients; develop and modify plans of care; provide rehabilitative treatment and therapeutic interventions; administer activities of daily living retraining, therapeutic activities, therapeutic exercises, modalities, and supportive or assistive devices; document evaluations, treatment minutes, progress, outcomes, and discharge summaries; provide patient and caregiver education; participate in patient care conferences, utilization review meetings, rehabilitation meetings, trainings, in-services, and conferences; obtain necessary durable medical equipment and supplies; and supervise or support occupational therapist assistants, rehabilitation aides, and students.
- Required Qualifications: Occupational Therapist license in Missouri; at least 1 year of Occupational Therapist experience; and long-term care or rehabilitation experience.
- Schedule: Day shift, 8 hours per shift, with a minimum of 40 hours per week.
