Ardor Health Solutions is seeking a Physical Therapist Assistant for a travel SNF/Rehab contract at Centralia Manor in Centralia, Illinois.
Centralia Manor is a skilled nursing facility located on the Liberty Village of Centralia campus. The facility provides skilled nursing and rehabilitation, including its Bounce Back Rehabilitation program, which focuses on restoring strength and improving daily function to help patients recover and return to greater independence.
Centralia Manor is a 120-bed skilled nursing facility, providing a substantial adult and geriatric rehabilitation environment.
The PTA will work under the direction of the Physical Therapist to carry out established plans of care for patients requiring short-term rehabilitation and ongoing skilled therapy.

The PTA will work primarily with an adult/geriatric population on therapeutic exercise, strengthening, gait and balance, transfers, functional mobility and patient education while working closely with the supervising PT and interdisciplinary rehab team.
