Required Qualifications: Certifications, Education Requirements, Degree Requirements, Location Requirements, and at least 1 year of experience in a SNF setting are required
Preferred Qualifications: Relevant specialized training or certifications in geriatrics or SNF environments are preferred
Additional Information: The role involves delivering patient evaluations, developing treatment plans, and documenting progress in accordance with facility policies and standards of care
