Job Summary:
Home Health Physical Therapist (SNF) position in the Charlotte area. This role involves providing therapy services to patients with chronic diseases, orthopedic conditions, and other diagnoses in a home health setting. The position offers a team-oriented environment with a focus on patient-centered care and outcomes.
Job Details:
- Shift: Days, 8 hours per day
- Schedule: Every 13 weeks with no weekend rotation, no call, and no major holidays required; RTO with manager approval
- Minimum Hours Per Week: 40 hours
- Start Date: 10/12/2026
- End Date: 01/09/2027
- Required Qualifications:
- One year of home health experience
- Homecare Homebase EMR experience required (no exceptions)
- Must possess a valid drivers license and proof of insurance
- PTA: BLS, North Carolina State License
- Preferred Qualifications:
- Experience with gait training, balance training, non-pharmaceutical pain management, and facilitating plans of care
- Good communication skills with the therapy and clinical team
- Ability to recognize signs and symptoms to prevent readmissions
- Experience with EPIC Rover and Samsung Note 9 phones
- Additional Information:
- Traveling primarily within Northern Charlotte, Huntersville, Cornelius, Davidson, Morrisville, Salsbury areas
- Approximately 6 visits a day, with an average mileage of 60-80 miles daily, reimbursed weekly
- Support provided by departmental leadership, schedulers, clinical managers, and peers
- Department culture emphasizes a team environment
