Description
American Traveler is hiring an RN Case Manager for a Utilization Review/Case Management role requiring NYS licensure, PRI certification, and 2 years of case management experience.
Details
- Day shift schedule from 8:00 AM to 4:00 PM
- Manages a caseload of approximately 14-18 patients
- Supports safe and timely patient discharge transitions
- Coordinates durable medical equipment (DME) and post-acute services
- Identifies high-risk patients and discharge barriers
- Collaborates with interdisciplinary care team members
- Documents and coordinates care using Epic EMR
- Preferred experience with Epic and CarePort transition tools
Requirements
- Active NYS RN license required
- NYS PRI Certification required
- Screen Certification required
- BLS certification required (AHA only)
- Minimum 2 years of relevant experience required
- Two professional references from previous managers required
- Skills checklist required for consideration
Additional Information
- Coordinates care planning and discharge logistics across an interdisciplinary team
- Uses Kronos for timekeeping
- Local candidates accepted within 60 miles at a reduced rate
- Candidates with prior work history at Northwell or Nuvance Health locations are subject to an eligibility review process before consideration
