Description

American Traveler is hiring a Registered Nurse Case Manager with NYS RN licensure, PRI certification, and 2+ years of case management experience to support discharge planning.

Details

  • Hospital-based case management role supporting safe and timely discharge planning for approximately 14-18 patients
  • Coordinates durable medical equipment (DME) and post-acute services for patients
  • Documents and coordinates care using Epic
  • Day shift schedule from 8:00 AM to 4:00 PM

Requirements

  • Current NYS RN license required (license required if applicable)
  • NYS PRI Certification required
  • Screen Certification required
  • BLS certification required (AHA certifications only)
  • Minimum of 2 years of relevant experience required
  • Experience with Epic and CarePort Transition preferred

Additional Information

  • Identifies high-risk patients and discharge barriers
  • Collaborates with interdisciplinary care team members to coordinate patient transitions
  • Local candidates within 60 miles accepted at a reduced rate
  • Two professional references rated standard or above average from a manager or above within the past year required for consideration
  • First-time travelers will be considered
  • Candidates with prior work history at Northwell or Nuvance Health locations are subject to an eligibility review process before assignment consideration
