Description

American Traveler is hiring an experienced RN Case Manager for a 13-week day shift assignment requiring a CA RN license and 2+ years of case management experience.

Details

  • Acute hospital setting within the Case Management unit
  • Day shift schedule, 5x8-hour shifts (8:30 AM - 5:00 PM)
  • Patient ratio of 1:20
  • Manages complex patients with chronic or complicated medical conditions
  • Coordinates with an interdisciplinary team to develop comprehensive discharge plans

Requirements

  • Active CA RN license required at time of submission
  • BLS certification required
  • ACM and Interqual certifications preferred
  • Minimum 2 years of case management experience required
  • Any charting system experience accepted
  • Community hospital experience preferred
  • Full date of birth and full SSN required for consideration
  • Two supervisor references preferred

Additional Information

  • Conducts initial patient assessments and ongoing stay reviews
  • Evaluates patient needs and coordinates discharge planning for complex cases
  • Works to ensure patients are discharged to the appropriate level of care
  • Candidates must reside within a 75-mile radius of the facility
  • RTO requests always require facility approval and must be submitted at time of consideration
  • First-time travelers are not accepted for this role
