RN Case Manager – Utilization Review | Bozeman, MT

Location: Bozeman, MT
Facility: Bozeman Deaconess Hospital
Contract: 13 Weeks
Shift: Days | 7:30 AM – 4:30 PM
Schedule: 40 Hours/Week + Weekend Rotations
Pay:$72/Hour on W2
EMR: Epic

Requirements

Active Montana RN License
AHA BLS/BCLS
Minimum 2 years of nursing experience
Must have Utilization Review experience
Acute-care hospital Case Management experience preferred
BSN preferred

Responsibilities

Develop and coordinate individualized patient care plans
Collaborate with physicians and multidisciplinary teams
Monitor patient progress and identify barriers to care
Assist with discharge planning
Perform comprehensive patient assessments
Coordinate patient services and promote continuity of care
Monitor and document care plans, interventions, and outcomes
