Requirements:
- Minimum 2 years of acute care Case Management experience.
- Experience in care coordination and discharge planning.
- Epic experience required.
- Experience with HMOs, IPAs, utilization review, and medical necessity assessments preferred.
- Acute hospital experience in ICU, Med-Surg, or Stepdown/PCU/IMC/Observation.
- Active California RN license required by start date. Pending license accepted.
Job Details:
- Patient caseload: Up to 25 patients per day.
- Patient ratio: 1:25.
- Weekend requirement: Every other weekend (Saturday and Sunday), with a weekday off.
- Time off: Maximum 7 days preferred; requests exceeding 7 days require client approval.
- Facility type: Acute Care Hospital.
- Dress code: Business attire or scrubs.
