Description

American Traveler is hiring an RN Care Coordinator for Case Management requiring an active MA RN license and Epic charting experience.

Details

Position within CCM Nursing unit
Manages a caseload of patients across the episode of illness from pre-admission to post-discharge
Coordinates care with an interdisciplinary team using patient assessments, care guidelines, protocols, and payer regulations
Day shift schedule from 07:00-15:30
Uses Epic EMR for charting

Requirements

Active MA RN license required; conditional or pending licenses not accepted
Both NURSYS and BON verification required for nursing license
AHA-issued certifications only
Flu vaccine required with no religious or medical exemptions allowed
Epic charting experience required

Additional Information

Coordinates care to support desired clinical and financial outcomes for assigned patients
Participates in ongoing evaluation of practice patterns and systems to improve quality, cost, and satisfaction outcomes
Mobilizes resources to maximize efficiency of care delivery
Local candidates residing within 60 miles of Boston are accepted at a reduced rate
No validated parking available at Boston locations; public transportation or paid public parking required
