Description

American Traveler is hiring an RN Care Coordinator for a Case Management position requiring an active MA RN license and Epic charting experience.

Details

  • Case Management/Care Coordination unit within a hospital setting
  • Manages a caseload of patients across the episode of illness from pre-admission to post-discharge
  • Day shift schedule from 07:00-15:30
  • Uses Epic for charting

Requirements

  • Active MA RN license required (no conditional or pending licenses accepted)
  • AHA-issued certifications only
  • Flu vaccine required with no religious or medical exemptions allowed

Additional Information

  • Coordinates care prescribed by an interdisciplinary team using patient assessment, care guidelines, protocols, and payer regulations
  • Participates in ongoing evaluation of practice patterns to improve quality, cost, and satisfaction outcomes
  • Mobilizes resources to maximize efficiency of care delivery
  • Local candidates must reside at least 60 miles from Boston to qualify for standard travel rate
