Description

American Traveler is hiring an RN Case Manager for a 10-week winter contract at a Level III trauma center, requiring 2 years of experience and Epic proficiency.

Details

  • Case Management unit within a short term acute care Level III trauma center with 252 staffed beds
  • Patient population includes med/surg, neurology, oncology, cardiac, and OBS patients
  • Day shift schedule, 5x8 hours, 8:00 AM-4:30 PM
  • Required to work every other weekend
  • 10-week assignment specifically for winter coverage
  • Part of the Yale New Haven Health System

Requirements

  • Active CT RN license or compact license required
  • 2 years of case management experience required
  • Acute hospital and home health experience required
  • Epic charting system experience required
  • Travel nursing experience required
  • 1 professional reference required

Additional Information

  • Responsibilities include care coordination, discharge planning, and collaboration with multidisciplinary teams
  • Documents pertinent information to facilitate care coordination and participates in care conference activities
  • Free parking available on site
  • No holiday RTO permitted and maximum of 5 days RTO allowed for this assignment
  • Local radius restrictions waived specifically for this 10-week assignment
  • Candidates with prior employment at the health system must have been separated for at least 2 years to be considered as a traveler
  • Flu vaccine required with no declinations accepted
  • COVID vaccine and booster required with only religious or medical exemptions accepted
