REQUIRED EXPERIENCE

• Complex Case Management RN experience required
• Experience managing medically complex inpatients required
• Experience with patients with complex medical conditions, socioeconomic challenges, and mental health comorbidities
• Experience with comprehensive case management and patient-centered care coordination
• Strong nursing assessment and care-plan development experience
• Experience with patient/family education, self-care coaching, motivational assessment, and adherence to care plans
• Experience with end-of-life planning preferred
• Epic experience preferred
• Education: BSN in Nursing or related field preferred/required, OR ADN with 5 years of Case Management experience

SCHEDULE

• 8-hour day shifts
• 8:00 AM–5:00 PM
• Monday–Friday

REQUIRED CERTIFICATIONS

• No certifications specifically required
• BLS not required

LICENSE

• Active RN license required at submission
