Position Overview

Responsibilities include:

Providing patient-centered care coordination and case management
Developing and monitoring patient/family care plans
Conducting nursing assessments
Supporting patient and family self-care and independence
Motivational assessment and coaching for care-plan adherence
End-of-life planning
Coordinating care with interdisciplinary teams
