POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans
MINIMUM REQUIRED QUALIFICATIONS – CA RN license; BLS; LA City Fire & Safety; 2 years recent acute hospital care coordination experience; Prior CommonSpirit/Dignity experience
LENGTH OF ASSIGNMENT – 13 weeks
SHIFT / HOURS PER WEEK – 8hr Days, EOW (40hrs/wk)
SYSTEMS – Cerner
