Collaborate closely with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe, efficient discharge planning. MINIMUM REQUIRED QUALIFICATIONS – FL/compact RN license BLS 2 years case management experience in an acute hospital setting Acute hospital experience in specialty during the past year with no more than a 90-day break Meditech & Midas experience PREFERRED QUALIFICATIONS – Complex discharge planning experience LENGTH OF ASSIGNMENT – 13 weeks SHIFT / HOURS PER WEEK – 8:00am-4:30pm SYSTEMS – Meditech, Midas START DATE – 9/15/2026 Facility Location A beautiful 150 acre Botanical Garden represents Florida’s extensive native and tropical plants and has amazed all those who go visit the garden.
