POSITION DUTIES - Coordinate patient care and discharge planning Complete medical, functional, and social assessments Develop individualized plans of care Identify and address barriers to safe, timely discharge Coordinate community and post-acute resources Collaborate with physicians, nursing, patients, families, and interdisciplinary teams Review admissions, continued stays, and appropriate level of care Support appropriate resource utilization and length-of-stay management Participate in interdisciplinary rounds and case conferences Advocate for patients and support quality outcomes Provide education on case management and discharge-planning processes MINIMUM REQUIRED QUALIFICATIONS - Active Montana RN or Compact License BSN Three (3) years of experience as a Registered Nurse is required, acute care setting required.
