Description

American Traveler is seeking an RN Case Manager for a day-shift position requiring 5+ years of hospital case management experience and an active ME or compact RN license.

Details

  • Hospital-based Care Management department with a 32-bed unit
  • Patient ratio of 1:20-24 with high acuity, complex cases and a high percentage of behavioral health patients
  • Utilizes EPIC, Wellsky, Careport/PASSR, Teams, laptop, and Ascom phone
  • Monday through Friday schedule with every third weekend and holiday coverage required
  • No on-call requirements
  • Floating required to cover Care Management department needs within a 25-mile policy between facilities

Requirements

  • Active RN license (ME or compact) required
  • Minimum of 5 years hospital case management experience
  • Minimum of 5 years hospital acute adult care nursing experience
  • Strong skills in discharge planning and care coordination
  • AHA certifications required prior to start if non-AHA certifications are submitted
  • Driver's license required for consideration
  • Two professional references required

Additional Information

  • Responsible for discharge planning and care coordination for high acuity, complex patients
  • Business casual dress code accepted; scrubs or professional casual attire permitted
  • Ideal candidates are flexible, easygoing problem solvers with a calm demeanor and can-do attitude
  • RTO should be minimal, with 3-4 days considered reasonable for a 13-week contract
  • Local candidates within 90 miles of any MaineHealth facility are eligible but are not eligible for renewals or extensions
  • Candidates who have worked at any MaineHealth facility within the past 12 months must disclose this information
